If you have already received your first vaccine shot, you should have received this CDC COVID-19 Vaccination Record Card from your public health care provider as proof of vaccination.
Make sure to keep it in.a safe place as it will likely serve as proof of vaccination which may be required for domestic or international travel soon.
We recommend the following:
Make a copy of your Vaccination Record Card and keep it at home
Take a photo on your mobile phone, favorite it, and e-mail it to yourself to keep a digital copy
Once your vaccination is complete (1 dose or 2 doses depending on vaccine) laminate your card at any office supply retailer, like Office Depot or Staples.
